Before you begin

Applicants who do not have an ASI membership number should apply for Associate / Affiliate Membership.
Associate / Affiliate Membership fee: ₹2,950 (including GST). Do not pay before submitting this form.
Keep these files ready: passport size photo (2 MB max) and, if available, your medical registration certificate (3 MB max).
After you submit, you will receive an Application ID and the payment details by email.
